This form is the first step in requesting a schedule change, do not use this form when requesting leave time.
Working hours are based on the times that library service is most convenient to the public.
Any change in one’s schedule must be approved by the department supervisor and must be in the best interest of the library, for clarification or further instruction, consult the employee manual.
Whenever an employee is absent from work through no fault of the employer, the employee will not be permitted to work solely for the purpose of restoring the time lost, and habitual tardiness is cause for dismissal.
This form is to be used to reschedule hours within one week, and it is not intended to carry over hours to another week nor allow for restoration of time lost in a previous week.
